Client Agreement

Client Responsibility
I freely consent to receive psychedelic therapy and/or somatic spiritual counseling from Rev. KP Psaki. I am responsible for my own growth, and I determine the effectiveness of the services rendered and the competency of the spiritual counselor. Psychedelic therapy and somatic spiritual counseling are not psychotherapy. My sessions with KP are not a substitute for psychotherapy, substance abuse treatment or medical care. I am 100% responsible for myself, my choices and behaviors. 

Confidentiality and Reporting
Information provided during sessions is confidential unless there is reasonable cause to know or suspect self-harm, that a child or elderly person has been subjected to abuse or neglect, or that a child or elderly person is being subjected to circumstances or conditions that would reasonably result in abuse or neglect. This is a professional relationship in which sexual intimacy is never appropriate. 

Insurance, Payment and Scheduling
I understand there is no insurance coverage for these sessions. Payment will be made within 48hrs of a session. If I must send a check, it will be sent within 24 hours after the session, and no further sessions will be conducted until payment is received by KP. There are no refunds after the completion of a session. If I need to cancel or reschedule a session, I will email KP with at least 24-hour notice at Rev.KP@waterandbones.org to avoid being charged for that time. If KP needs to cancel or reschedule a session, she will provide at least 24 hours notice. A full fee will be expected for a late-start/shortened meeting.

Acknowledgment and Release of Liability
I acknowledge that I understand the above information. Furthermore, I agree to hold harmless Rev. KP Psaki from all liabilities and claims which may arise as a result of my participation in spiritual counseling.
